In compliance with Indiana Code 4-1-6, applicants are hereby notified that all required information must be provided in order for the application to be processed. Failure to provide the required information will result in the application not being processed.
 
Applicants have the right to challenge, correct, or explain information maintained by the agency. Information submitted may become part of the public record.
 
Examination scores and transcripts are considered confidential and will only be released when required by law. In such cases, applicants will be notified as required.
 
 
 

Mandatory Disclosure of Social Security Number

In accordance with Indiana Code 4-1-8-1 and 25-1-5-11(a), disclosure of your Social Security Number is required by the state agency processing your application.
 
Providing your Social Security Number is mandatory. The application cannot be processed without it. Failure to provide this information will result in denial of the application. Application fees are non-refundable.
 
 
 

Application Expiration (Abandonment Policy)

If an applicant does not complete all required steps within one (1) year of submitting the application, the application will be considered abandoned without action by the Board.
 
Any application submitted after abandonment will be treated as a new application, including all required fees and requirements.
